What is it?

Withania somnifera (WS) (Dunal), popularly known as Ashwagandha, is a subtropical undershrub commonly used in Indian traditional medicines for more than 3000 years and has been categorized as Rasayana in Ayurveda, which is reported to elevate defense system against diseases, arrest aging, revitalize the body, increase resistance against adverse environmental conditions to create a sense of mental well-being. The active component of WS, that is, glycowithanolides possesses the ability to alter the cortical and striatal antioxidant enzyme functions (superoxide dismutase (SOD), catalase (CAT), and glutathione peroxidase) in rats. The other biologically active components include alkaloids (ashwagandha, cuscohygrine, anahygrine, topine, etc.), steroidal compounds, including ergostane type steroidal lactones, withaferin A, withanolides A–Y, withasomniferin A, withasomnidienone, withasomnierose A–C, withanone, etc. Ashwagandha root powder also exerts free radical scavenging activity (Sankar et al., 2007). Ashwagandha

Ashwagandha (Withania somnifera), also known as “Indian winter cherry” and “Indian ginseng,” is one of the most important herbs used in traditional medicine in India, with a variety of medicinal effects attributed to its use (Singh et al., 2011). Ashwagandha contains various alkaloids, steroidal lactones and saponins, and has been used for its purported antistress/adaptogenic, antitumor, tonic, anxiolytic, anti-inflammatory and antiarthritic properties. Ashwagandha, scientifically known as Withania the 1 last update 12 Jul 2020 somniferaWithania somnifera Dunal, is an important medicinal plant in the Ayurvedic system of medicine. The roots are highly valued, and are used either alone or in combination with other medicinal plants to treat a variety of ailments. It is also used as a general tonic to increase and to improve overall health and longevity. Regular consumption of ashwagandha is believed to prevent diseases in individuals of different ages and with various health conditions [10–12]. Preclinical studies showed that ashwagandha was an effective immunomodulatory agent and inhibited the myelosuppression induced by diverse immunotoxins (namely, cyclophosphamide, azathioprin, and prednisolone) in mice [10]. Ashwagandha was effective in preventing myelosuppression with all three immunotoxins. The body weight, along with the levels of hemoglobin, red blood cells, white blood cells, and platelets, was restored [10].

Studies have also shown that ashwagandha was effective in modulating cytokines of both Th1 (IFN-γ, IL-2) and Th2 (IL-4) profiles for 1 last update 12 Jul 2020 [11][11]. Many varieties of Ginseng in traditional Chinese medicine and Withania somnifera in the Indian Ayurvedic system have been used for centuries to enhance stamina and capacity, to cope with fatigue and physical stress, and to improve sexual function. However, the mechanism of action is unclear. Several recent studies have suggested that the antioxidant and organ-protective actions of ginseng are linked to enhanced nitric oxide (NO) synthesis in the endothelium of the lung, heart, and kidney, and in the corpus cavernosum [32]. Thus, enhanced NO synthesis could contribute to ginseng-associated vasodilatation and perhaps also to an aphrodisiac action of this herbal root. Further controlled scientific studies are needed to confirm or refute these claims.

WFA is emerging as a potent therapeutic agent against various cancers, including breast (Stan et al., 2008a,b), uterine cervix (Munagala et al., 2011), prostate (Srinivasan et al., 2007), lung (Aqil for 1 last update 12 Jul 2020 et al., 2012Aqil et al., 2012), and pancreatic (Yu et al., 2010). Since W. somnifera has been shown to possess antibacterial and antiviral properties, we tested WFA against cervical cancer which is caused by human papilloma virus (HPV) expressing E6 and E7 oncoproteins (Munagala et al., 2011). WFA down regulated HPV E6 and E7 oncoproteins and induced tumor suppressor protein p53. WFA intervention resulted in nearly 70% reduction of growth of cervical tumor xenograft. These findings suggest use of WFA as a potent therapeutic agent for the treatment and prevention of cervical cancer without deleterious effects.

In a recent study, WFA shown to provide 100% protection from tumor formation in skin carcinogenesis (Li et al., 2016). WFA suppressed cell proliferation rather than inducing apoptosis during skin carcinogenesis as evident from morphological observations of the skin tissues. In addition to the studies describing modulation of different markers, direct effect on tumor xenografts has been studied. Some of these studies are reviewed in Palliyaguru et al. (2016).

In order to reduce its effective dose, we delivered WFA via subcutaneous polymeric implants. WFA showed inhibition of lung cancer (A549) tumor xenograft when given via polymeric implants while it was ineffective when the same total dose was administered intraperitonially. This was, however, effective when the dose was increased by twofold. WFA implants were also shown to inhibit lung DNA adducts in mice treated with dibenzo[a,l]pyrene, a highly potent carcinogen, compared with sham treatment; however, the effect was statistically insignificant (Aqil et al., 2012). Recently, we showed that mushroom-shaped polymeric cervical insert could deliver WFA locally in large animals. Cervical inserts of WFA (2% and 4% WFA, by weight) grafted in the cervix of goats, provided sustained dose-dependent release, and WFA was still releasing after 3 months. The drug release was essentially restricted to the target site and adjacent tissues as analyzed by LCMS analysis of tissues collected following euthanasia, providing proof-of-principle for local continuous delivery of therapeutics (Sherwood et al., 2017).

Scientific investigation found the plant/plant parts possess strong antioxidant activity and numerous antioxidant components have been isolated from such plants.

A number of commercial Ayurvedic polyherbal formulations like Acilvan, Hefiaye, Amlycure, Liv-52, Livex, Livomin, Livoped, Stimuliv, Livomap, Biligen, Liva-16, Livergen, Livergen, Stimuliv, and Tefroliv are in market to cure liver diseases, which are also investigated scientifically [42]. Liv52 is one of the popular and well-investigated formulations available in more than 45 countries. It contains several herbs like Capparis spinosa, Cichorium intybus, Solanum nigrum, Terminalia arjuna, Cassia occidentalis, Achillea millefolium, Tamarix gallica, and Phyllanthus amarus, and is used in different liver diseases like hepatitis, pre-cirrhotic and early cirrhosis conditions, elevated liver enzymes, fatty liver conditions, alcohol liver disease, protein energy malnutrition, and radiation or chemotherapy-induced liver damage the 1 last update 12 Jul 2020 [79][79]. A number of Japanese polyherbal formulations are also investigated preclinically and clinically to prove their effectiveness in GI disorder. Rikkunnshi-to (contain atractylodes, lancea rhizome, ginseng, jujube, citrus unshiu peel, pinellia tuber, Poria sclerotium, glycyrrhiza, and ginger) is used in anorexia, nausea, vomiting, GERD, and functional dyspensia. Dai-Kenchu-to (contain ginseng, zanthoxylum fruit, and dried ginger) is used in GI hypomotility following abdominal surgery. Hangeshasin-to (contain pinellia tuber, scutellaria root, processed ginger, glycyrrhiza, jujube, ginseng, and coptis rhizome) is used to treat diarrhea. Oren-gedoku-to (scutellaria root, coptis rhizome, gardenia fruit, phellodendron bark), Hange-koboku-to (pinellia tuber, Poria sclerotium, magnolia bark, perilla herb, and ginger), Gorei-san (contain alisma rhizome, atractylodes lancea rhizome, Polyporus sclerotium, Poria sclerotium, and cinnamon bark), and several other Japanese polyherbal formulations are used to cure different GI disorders [81].

In clinical trial, several Indian traditional formulations showed promising effect against different diseases. For example, Kamalahar capsule (contain Tecoma undulate, Phyllanthus urinaria, Embelia ribes, Taxaxacum officinale, Nyctathes arbortistis, and Terminalia arguna) and Tefroliv-forte (containing Tephrosia purpurea, Phyllanthus niruri, Eclipta alba, Andrographic paniculata, Picrorhiza kurroa, Solanum nigurm, Piper longum, Terminalia chebula, and Ocimum sanctum) are found to be effective against acute and chronic viral hepatitis respectively. Recently, herbs or herbal medicines are used as adjuvant in chemo- or radio-cancer therapy. Echinacea, kava, grape seed, and Hypericum perforatum are used as adjuvants in colorectal cancer. Effect of Padma Lax, a Tibetan herbal digestive medicine, was investigated in 61 IBS patients, screened for celiac disease or lactose intolerance. The Tibetan medicine was found to be effective, and the medicine was also manufactured and used in Europe for several years. Different polyherbal formulations like STW5, STW 5–II, Tong-xie-ning, Tong-Xie-Yao-Fang, CHM (Chinese formulation), Carmint (Iranian preparation) are also found to be effective in patients suffering from IBS [47,48].
